This happens before the operation

Many women suffer from their big breasts. The breast is big, heavy and hangs, the patient does not feel well with her chest, suffers from back pain, the bother disturbs in everyday life and in sports. I have a lot of experience after a thousand operations in the thoracic surgery and work on the 4D concept. This means an accurate analysis of surface changes, volume depletion or loss, gravity and proportions.

After a comprehensive initial discussion, in which desire and possibilities are debated, the conclusion can be represented by means of a 3D planning of the “desire bosom”. The patient sees in the 3D scan exactly how her breasts might look after the operation, gets security and can imagine the result better. One can see exactly which implant fits best to one’s own body and can look at one’s own breast in advance after the operation. It is also important to show what is “not possible”. We are 100% honest in advance and do not unrealistic expectations.

There are findings that have a medical indication – for example, if the big breast is proven to cause significant physical discomfort. If this is the case, the costs for a breast reduction are partially or completely covered by the health insurance. For many health insurances, the minimum amount of fat and gland tissue to be removed is 500g per breast.

This happens during and after the operation

To reduce the volume of the breasts, glandular fat tissue is removed and the breast is remodeled. Breast reduction is usually done with a T-cut, as both skin and glandular tissue must be removed. The breast reduction is performed in a light general anesthetic. The surgery itself takes 1.5 – 2 hours.

Breast reduction surgery takes place in Vienna on a daily basis and in general anesthesia. Overnight stays at the clinic is rarely necessary today for medical reasons. At discharge, you will receive a support bra that you should wear for 6 weeks. One will be able to work again after 1 to 2 weeks, but should refrain from sports activities in the first 6 weeks. How long do I drop out?

Breast augmentation surgery does not take longer than 60 minutes. You can go home the same day. There is no need for an overnight stay unless by special request. The first days after the breast augmentation should be sparing. If you do not have a physically demanding job, you can usually work after a week. Sport is not allowed for 6-8 weeks, as the implants must heal in the position at rest.

How strong will scars and pain be?

We work with a gentle surgical technique with radiofrequency, which results in less tissue injury. Result: faster healing, less pain. There are no more drainages necessary. Scars that are set are always visible. Breast augmentation can lead to scarring in the lower breast crease, which fades to 99% very well. In 10 years I treated 2 patients who developed a very noticeable scarring. The scar around the areola, which is mainly carried out in the case of tubular breasts, is usually less noticeable as well, so there is a difference in contour between the darker skin of the areola and the breast skin. In about 10% of the cases, however, we see a scar formation which can be red for several months and must be treated with laser and cortisone.

What are the risks and how do you deal with complications?

The surgical transection of the pectoralis major (= large pectoral muscle) at the lower approach can cause bleeding – but these are immediately breastfed intraoperatively. Complications that may occur as part of breast augmentation include rebleeding, infection, capsular contracture and hypertrophic – excessive – scarring. However, the most modern and gentle surgical techniques make it possible to minimize the risk of these complications. It is important that in case of complication as a doctor you know how to handle it. After more than 1,000 breast operations, we are aware of all the complications and have successfully treated every possible one.

What kind of anesthesia will I have?

Breast augmentation surgery takes place in Vienna on a daily basis and in general anesthesia. It is a mild anesthetic in which you sleep completely, but wake up immediately after the end of the operation. Overnight at the clinic is no longer necessary for medical reasons. Our patients like to stay the night when the journey is far away or you just want to spend a night rest from everyday life.

Has there ever been any complications with anesthesia in your surgery?

No. The risk of anesthesia is determined individually for each patient by our team as part of the anesthesia ambulance. We use a very short-acting anesthetic, which allows the patient to wake up a few minutes after the anesthesia and leave the clinic a few hours after the procedure painlessly and painfully. In principle, our anesthesiologists are in close contact with the AKH Vienna, which is located next to the private clinic Währing, it should come after all these years, nevertheless, a complication during anesthesia.

May an accompanying person be present before / after the operation?

Yes! After about 1 hour of care in the recovery room, all patients come to the ward, where they can then receive a visit in their room.

How long in advance is the surgery appointment agreed?

The waiting time for an OP appointment is usually around 4 weeks. Normally the patient comes for the first consultation and – if she chooses to have an operation – an appointment for surgery is reserved. At least 15 days before the surgery, there will be a meeting with the anesthesiologist and Dr. med. Rolf Bartsch personally to explain the procedure again and to answer any questions. What does the aftercare look like?

Further follow-up appointments in our surgery are important to accompany the healing process optimally and are agreed with you before the procedure. After 2-3 days it comes to plaster change, after about 1-2 weeks, depending on the technique, for suture removal. Until the suture is removed, avoid exercise and do not wet the sutures.

When is the risk of capsular contracture?

The possibility of capsular contracture is very rare. The rate of capsular fibrosis is below 0.5% in our patients. Whenever a patient meets, the question arises “why me”? The reasons for capsular contracture are not clear. Neither she nor the doctor are to blame. It is important that depending on the stronger fibrosis – this is divided into 4 stages after BAKER – must be traded. The costs for a necessary implant replacement are around 3,900 € and are not covered by any public health insurance.

I still have a child wish. Will my breastfeeding ability be preserved?

Yes, the ability to breastfeed is preserved. You can usually breastfeed your baby without any problems. Should the desire for a child be within the next year, we advise to wait with an operation. Especially with tubular breasts, the ability to breastfeed is impaired even without surgery. In some techniques, such as cutting through the areola, the ability to breastfeed may be compromised. The connection of the nipple to the mammary gland tissue is never severed by modern techniques.

What technique do you use in the operation?

The technique depends on the initial situation. If the breast is round but just too small it is usually enlarged by a cut in the lower breast crease. In the case of tubular breasts, access via the areola often makes sense, because the change of the form and not only the enlargement is in the foreground. The position of the implant is usually under the muscle. This creates an inner bra that supports and protects the implant. The exact preparation of the implant pocket leads to less tissue damage and faster healing. The cavity is thus customized for each patient.

Will the breast have a higher sitting overall?

In the beginning yes. The first 6 – 8 weeks, the breasts are swollen and harder after each breast augmentation. Over time, the tissue swells, and as a result, the initially stronger projection disappears in the upper half of the chest. The implant is always used so that the maximum projection is just behind the nipple.

How will my breasts look after the procedure? Do you work with trial sets of implants or a 3D simulation so I can see what my chest looks like after the procedure?

We have a state-of-the-art 3D simulation. With the help of an infrared camera, the own breast is scanned and simulated. With a software, only the size and type of the implant as well as the position can be determined. The 3D simulation can make static and moving images. The simulation can be turned in all directions and one can estimate exactly what the desired bus could look like. Whether from the side, from the front or from above – from every angle we get a good visual idea, whether the chest will suit you or not. You get an online access to the simulation of the breast enlargement and can look at home in peace and quiet.

There are already slight stretch marks on my chest, does the OP affect them?

Normally, after surgery, there are no stretch marks on the chest, as we choose the implants to respect the anatomical limits as well as the extensibility of the skin. If strong stretch marks are already present in advance, they can turn red in rare cases. In this case we should not plan a “too big” enlargement together.

What about weight gain / weight loss? What effect do these have on my operated breast?

You should avoid weight fluctuations of more than 5kg after breast augmentation, as these may cause the breast to hang faster. During growth and declines, the supporting tissue of the breast is stretched and thus the shape of the breast can change.

Which implants do you use? From which manufacturer?

I use the latest generation of silicone implants made by the German company Polytech, Motiva and B-Lite Implants. This new generation of silicone implants is lighter, more forgiving, made of a more dimensionally stable gel and is more robust. They are characterized by a high standard of quality and safety. The latest implants integrate better into the female breast, the silicone gel adapts to the natural movement of the body.

The advantages of B-Lite implants?

The mixture of microspheres and silicone gel makes the implants not only 30% lighter, but also firmer. This avoids an unwanted “rippling” effect. This effect occurs when implants make waves that can be seen through the skin surface. The stronger material of the B-Lite implants prevents this annoying “folds”. But the altered structure in the silicone gel also means that the breasts feel firmer and a little less natural.

What is Rippling?

Breast implants differ not only in their shape – round, semicircular or anatomical – but also in the strength of the gel that is used. The softer the gel, the sooner it can make waves. Especially if the patient is particularly thin or the breast tissue is very loose. Especially after pregnancies and the subsequent lactation, the mammary gland tissue is reduced and thus the rippling risk is higher for soft implants. Modern implants have less rippling. Anatomical implants are usually less than round, as they consist of a stronger gel. B-Lite implants show virtually no rippling, but can be felt more firmly.

Shape of implants: Should I take round or anatomical implants?

There are implants for more push-up effect (round implants) or anatomical implants for very natural results, a flatter cleavage and a bulbous lower pole produce. Above all, the projection of the implant is crucial, whether the breast looks large or very large. Round and semi-circular implants (these are simply round implants with less projection) have 50% of the volume above the tip and 50% below. Depending on the manufacturer, anatomical implants have 60-65% of the volume in the lower pole and 35-40% in the upper pole. This makes the décolleté simply more natural.

Are the edges palpable if the implant is too wide?

Yes, that is possible. Therefore, when choosing the implant, we pay attention to respecting the anatomical limits of the breast. Too wide implants have a further disadvantage: over the years, a high pressure on the adjacent mammary tissue. As a result, blood circulation is restricted and mammary gland tissue dies. The effect is that after a few years you only feel the skin and implant between your fingers. The edge of the implant can very often be felt in patients with little intrinsic tissue in the lower breast crease. This is not a cause for alarm, but is explained by the fact that the implant below is not covered by the muscle, and thus is easily noticeable.

Dual plans or submuscular?

Depending on the anatomical conditions we choose the appropriate preparation technique: usually this is often the Dual Plane method, which provides a particularly natural result: in this case the implant is covered in the upper part of the breast muscle, in the lower part it is loosely embedded in the glandular tissue. What’s up with 4-musekl technology? This is a situation completely under the muscle. Since the mammary gland and implant are completely separated, the result often feels fake.

Can implants slip because they do not hold muscle at the bottom or how is that prevented?

No, we prepare the implant pocket so that the implant fits exactly and does not slip – at the top it is always covered by the muscle, and thus sits stably in the right place. It is important to reassemble the layers correctly after surgery and to form an inner bra in the lower breast crease. In the past 10 years, we have already seen that implants give way down even years after the breast enlargement. In that case, the lower breast crease must be reconstructed in a small procedure.

Does the breast fold have to be relocated?

No. We choose the implant so that the natural base of the breast is not exceeded – so the lower breast crease does not need to be displaced. Only in very rare cases, where there is hardly any own tissue, you have to lower the lower breast crease. In recent years, we have corrected many patients whose undercreases have been set too low.

Is it possible to fix the final size in ml in the surgery contract?

We discuss the final size in our OP conversation and again on the day of the operation when marking. Because you only have the actual starting position during intraoperative surgery, it may be that you choose an implant size smaller or larger – but the differences are so minimal in the projection that this objectively no effect on the discussed or shown in the 3D scan end result Has. If we make changes during surgery, these are in the range of 5-10% of the volume. So do not worry about a surprise. It happens exactly what was agreed.

Does the insurance pay anything?

Are corrections included in the price?

Changes in shape due to pregnancy / breastfeeding?

Of course, with silicone implants, breast size increases during pregnancy and lactation. Above all, long breastfeeding can cause the breast to hang more afterwards, which may require a new operation and possibly tightening of the breast. The more breast tissue is present and the longer breastfeeding lasts, the greater the changes in the breast can be.

Do I have to do a mammogram before surgery?

No, mammography is usually not necessary before breast augmentation. We recommend it only to those patients who have painful knots or similar before surgery. want to clarify in the chest. But you should definitely do a mammogram if you are over 35 years old. This as part of the classic breast cancer screening.

Can I have a BE if there is breast cancer in my family?

If you have any known cases of breast cancer within your family, we recommend that you take a blood sample to rule out a genetically increased risk for breast and cervical cancer (BRCA 1 or BRCA 2). Then we can individually make the right decision for you. In patients over the age of 35, we recommend ultrasound and breast mammography before the procedure.

Do I have to buy cool packs for after surgery at home?

No, you do not have to. Immediately after the operation, a thin tape bandage is applied over the chest to reduce swelling. This should stay on for 5-7 days.

How long do I have to wear a support bra?

At discharge, you will receive a support bra that you should wear for 6 weeks.

When is what kind of sport possible again?

Since the pectoral muscle is released only at its lower edge, no permanent loss of strength is to be expected – after a 6-week break after surgery, all sports activities can be performed again.

What do I have to look out for in the sun and solarium?

Direct sunlight and solarium should be avoided as long as the scars are red. It could lead to dark scars.

When can I sleep on my stomach again?

On the side after a few days, on the stomach after a few weeks.

Is it possible to wear a push-up bra with an operated breast?

Yes you can.

Do I get antibiotics after surgery?

No, our anesthesiologists already administer the antibiotic during the operation, so that no more antibiotics have to be taken afterwards. In rare cases, we secondary operations, it may be that a gift over several days makes sense.

Do I have to inject a remedy for thrombosis?

No, since you are not bedridden after breast augmentation but should (and should) move normally, no thrombosis injections are necessary.

Should I take glutamine for wound healing?

No, you do not have to take any wound healing remedies after the operation. However, you should stop smoking one week before surgery and for up to 3 weeks thereafter, as this has a negative effect on wound healing.

Should I make a lymph drainage appointment after surgery?

Lymph drainage has the advantage that swelling after surgery can disappear faster. We offer this possibility through our ordination, which makes sense when most within the first 14 days.

Do I get drainages?

No, we normally do not use any drainage in breast augmentation, as we already carefully and conscientiously stop bleeding intraoperatively. Even internationally, hardly any drainages are used for breast augmentation. The reason for this is that they can not prevent rebleeding, cause severe pain after surgery, and bleed when drainage is removed. The advocates believe that the vacuum causes the contact between the implant and the tissue to become faster, thereby reducing the danger of seroma (formation of tissue fluid). Due to the careful operation of our patients breast augmentation, we almost never see any seromas. Even without the successful use of drainage for over 3 years.

How long does the plaster stay up?

After 2-3 days it comes to plaster change. In the technique of the areola, the sutures and patches are removed after 7 days. When cut in the lower breast crease after 14 days.

When will the seams be removed?

When may I have sex again?

After 6 weeks without hesitation. This is the time when the implants are healed with a delicate cup and the risk of “slipping” is thus very low. Within these first 6 weeks of healing, you can do everyday activities, but should not do sports that strain the upper body.

When may I take a shower again?

The suture removal takes place between the 7th and 14th day after surgery, depending on the suture technique used. Only now you can shower again with the consent of the doctor. Please use ph-neutral shower gel due to the sensitive scars.

At what intervals do the control appointments take place after the surgery?

Which scar ointment do you recommend?

We recommend the scar cream AND Repair and Protect and Bionect Hyaluron Gel. It is important to care for the scars until they completely fade and are no longer red. If the scar turns white, it will not change much. That means from this point on you can not make any big changes with the scar care.

Have there been any cases or requests that you have declined?

Yes, of course, whenever the wish does not match the possibilities. Most of the time, however, it is not a “rejection”. Most of the time, patients understand why something is not possible.

How long does one have to wait for a breast augmentation?

The waiting time for an operation is about 2 weeks. For the surgery date about 2-3 weeks. But it depends on the season. It can be a little longer in winter.

Cost of a breast lift without implants?

Can the scars be removed / treated after a certain time?

We always treat scars as soon as the threads are removed. Scars NEVER disappear but usually become less noticeable. Lasers, silicone preparations, creams etc. can accelerate this.

Opinion on Motiva implants?

Motiva implants have only recently been on the market and I see no particular advantage compared to the polytech implants that we have been using successfully for years. The topic with the nano surface is overrated. It is more the technique that prevents capsular fibrosis.

Can anything shift with a bra with underwire?

A bra with underwires should not be worn until 6 weeks after the operation, since the implant can still be pushed around.

What needs to be considered when enlarging a tubular breast?

A tubular breast arises because the breast has not fully developed. Therefore, the focus of the operation is less on enlargement than on changing the shape. If you have changed the shape of a pointed breast in a round breast, you can enlarge it. Most times with a smaller implant.

Is the breast always modulated again during a breast lift? Is “pulling up”possible?

Tightening cannot be meaningfully combined with autologous fat in the same procedure. Because the breast also needs incisions in the gland when tightening. The fat cannot heal there.

Are there reputable insurance policies that cover a possibly failed breast enlargement? Do you have any experience?

Are there permanent methods without silicone?

Silicone implants do not have an “expiry date”. An alternative to this is ONLY autologous fat. Most of the women who want more volume in the breast are very slim, which means that there is little autologous fat and therefore this method is not possible.

Implant in front of or behind the pectoral muscle?

Lots of own tissue: implant in front of the muscle because you don’t have to hide it.
Little own tissue: implant behind the muscle, otherwise you can see the edge of the implant through the skin.

What do you have to consider with a second enlargement?

The breast implant should never be wider than the breast base. If only higher. The bigger the implant, the more likely it is that there will be complications.

Tightening again and changing the implants? Cost point?

A breast lift and new, round implants cost around € 6,500, including hospital, anesthesia and all controls. Anatomical implants surcharge approx. € 800.

Does the fat not degrade when enlarged with autologous fat? For example in sports?

After breast augmentation with autologous fat, a large part of the fat cells grow. After 3-6 months you can see the end result. Exercise or weight loss can make fat cells smaller, but they don’t go away.

How long after weaning could a tightening or enlargement be carried out?

After weaning we recommend 6, or even 12, months to wait for breast surgery. Above all, an increased risk of infection from the active mammary glands can become a problem.

After how many years should the breast implants usually be replaced?

Breast implants have no expiration date. I think you might have to think about a change after 20-30 years. With modern ultrasound you can check from the outside whether the implants are intact. As stated earlier, a change after 10 years is no longer necessary.

Can you make a nice, round shape from a tubular breast?

Yes, in any case. In the case of the tubular breast, the change in shape is in the foreground. An implant is only necessary if there is also a lack of volume after tightening. This operation leads to an enormous improvement in the quality of life.
