VIRTUALLY NO PAIN 24 HOURS AFTER BREAST SURGERY. HOW IS THAT POSSIBLE? WE EXPLAIN HOW:
Recently, a term has manifested itself and has even almost become a brand – the “24h breast”. But what is behind it, what does a “24h breast” mean? The term stands for the subsequent freedom from pain and movement after breast surgery – breast lifts with and without implants, breast reductions and breast augmentations.
Many women cherish the desire for a larger, smaller, firmer, in any case for them more beautiful bust. But the fear of pain, of a long downtime and restriction of movement keeps many from an operation. With the 24h breast, one is not bound to any season, the operation does not necessarily have to take place in winter, but can also be performed in late spring, in order to be bikini-ready in summer. I have many patients who, if they had known how pain-free they would be afterwards, would have fulfilled their desire for the dream bust much earlier.
In my last Insta-Story under #doc.rolf we filmed a breast augmentation and showed afterwards how the patient feels and how mobile she is.
Because a 24h breast means full mobility and almost freedom from pain 24h after surgery.
How does this work? The interaction of the following factors is decisive:

3D simulation
If there is a desire for a breast lift or breast augmentation, we discuss all details, wishes, possible risks, the procedure and aftercare in a comprehensive consultation. By means of a 3D simulation on www.3dbrust.at, the patient can see in advance how her breast may look after the operation with the desired implants. This gives the patient not only confidence and security, but also an idea and anticipation of her new breast.
Implants
The choice of implants – anatomical or round, as well as the size of the implants are the most frequently asked questions of my patients. What many people do not know is that not every implant size can be used for every breast. The choice of breast implant depends on the width of the breast base. The width of the implant cannot be larger than the breast base.
It is important to respect the natural breast base. There are patients with narrower or wider breast bases.
How much space is there in the breast? You can enlarge the breast until you reach the elastic level. Then you get to a plastic level, if you take out the implant from the breast, it is already overstretched. Enlarged even further, beyond the point of no return, it comes to the phase of deformation. The breast becomes much too expanded.

Are implants placed in front of or behind the muscle? I place implants behind the muscle 99% of the time. This way the implant is supported as if by an inner bra and the cleavage has a natural transition. Also, the durability of the shape is much longer because the weight of the implants is not carried by the ligamentous apparatus of the breast alone.
If the implant is too wide, the breast will be unnaturally overstretched to the side.
Do implants have to be replaced? The newest, modern implants have no expiration date, I advise to check the implants every 2-3 years by ultrasound.
Anesthesia
Our very experienced and professional anesthesia team clarifies the basic and important things with the patient in advance in the anesthesia outpatient clinic. The team administers painkillers before, during and after the operation without the patient having to ask for them. This is also a decisive factor in ensuring that the patient is and remains pain-free immediately after the operation.
Surgical technique
I operate atraumatically, which means tissue-sparing. A technique in which as little tissue as possible is injured. We use a so-called current needle, which makes the incisions similar to a laser beam and thus gently prepares the tissue and the implant pocket. This results in less damage to nerves and vessels and less bleeding, which in turn results in fewer inflammatory cells and therefore less pain.
Dressing application
We use a combination of a specific tape bandage and a support bra that is not too tight. Drains, which can also cause severe pain, we have been using for several years now only in very few cases. We do not normally use drains in breast augmentations because we already stop bleeding very carefully and conscientiously intraoperatively. Even internationally, drains are rarely used in breast augmentations. The reason for this is that they do not prevent postoperative bleeding, can cause severe pain after the operation, and can cause bleeding when the drains are removed. Proponents believe that the vacuum allows contact between the implant and tissue to occur more quickly, reducing the risk of seroma (formation of tissue fluid). We almost never see seromas in our patients due to our gentle technique.
RESULT
A successful 24h breast surgery shows how well all factors have worked together during the surgery: the work of the surgeon, the surgical technique, the work of the anesthesiologist, the bandaging.
The result: the patient can lift her arms and stand up already 2h after the surgery. After 24h she is completely mobile and almost painless. She has only the feeling of a stronger muscle soreness.
CONCLUSION
The 24h breast really exists, patients no longer have to expect protracted pain, downtime or immobility after breast surgery. The only thing that matters is the perfect interaction of factors.
